Family Care/IRIS/Partnership
Information & Assistance Specialists are available at the ADRC of the Lakeshore to assist individuals and their families in information regarding long-term care programs and other options that may be available. Eligibility requirements do apply. The ADRC shall administer a Long-Term Care Functional screen to determine functional eligibility into a long-term care program. Information regarding financial eligibility guidelines will be provided. More information on the programs is listed below.
