TRANSPORTATION FOR SENIORS AND INDIVIDUALS WITH DISABILITIES
The ADRC of the Lakeshore is your resource for transportation opportunities if you meet one or more of the following criteria:
- Age 60 and older
- An adult with a temporary or permanent disability
- Must reside within Manitowoc County
- No access to transportation
Transportation services are available for medical appointments. Rides can also be provided for shopping, employment, and social and personal business purposes based on available funding. This program is funded in part by the Federal Transit Administration (FTA) as authorized under 49 U.S.C. Section 5310 Mobility Options of Seniors and Individuals with Disabilities Program (CFD 20.521).
COORDINATED PUBLIC TRANSIT HUMAN SERVICES TRANSPORTATION PLAN
A Coordinated Public Transit Human Services Transportation Plan is a five-year plan dedicated to assisting the elderly and people with disabilities to receive specialized transportation based on their individual mobility needs.

REASONABLE MODIFICATION ACCOMODATION REQUESTS
Passengers with disabilities may request modifications to current service procedures to access the service as long as the request:
- Does not fundamentally alter the service.
- Does not create a direct threat to the health and safety of others.
- Is not necessary to permit the passenger to use the services for their intended purpose (non-discriminatory).
Such modifications should be made in advance to properly consider and plan for such modification.

TRANSPORTATION – MANITOWOC COUNTY
The ADRC of the Lakeshore contracts with various transportation agencies to help meet your transportation needs based on eligibility.
Maritime Metro Transit’s One-Stop Call Center serves as an information referral service for individuals seeking transportation options. Manitowoc’s Mobility Manager is on hand to provide individualized transportation information by calling (920) 686-6977.
Hearing impaired customers can contact ADRC of the Lakeshore or Maritime Metro Transit through the Wisconsin Telephone relay system line (1-800-947-3529).

Fares within Manitowoc and Two Rivers for elderly (65+) and disabled are $.75 each ride or $30.00 for a one-month pass. Adult fare is $1.50 each ride. Purchase of a monthly pass includes discount privileges or special offers at area retailers.

Transportation for City of Manitowoc and Two Rivers residents who are not physically able to utilize the bus service. ADA certification is required. Copay of $3.00 for one way ride for curb-to-curb service. Door-to-door service will be provided at a cost of $5.00 each way. This transportation service is provided by Assist-To-Transport. Transportation services to and from medical appointments, hospitals, nursing homes, etc. - Assist-To-Transport
Provides transportation for elderly and disabled residents. Medical & dental appointments, grocery stores, and nutrition sites are common destinations. It is necessary to pre-purchase tickets; all riders will be required to give the driver a ticket.
Provides city ADA and rural disabled transportation. Provides rural transport to sheltered employment, medical appointments, and others as necessary. Co-payment is required for transports. Call for price information. (920) 682-8820

Provides transportation within the city of Two Rivers only for ambulatory Senior Center members age 50 or older.

Manitowoc County residents only. Eligible ride types include employment, education, health, food, legal, housing, and economic stability. Rides must be scheduled in advance (1 day is preferred). The ADRC of the Lakeshore now provides transportation to rural elderly or disabled individuals in Mishicot, Valders, Whitelaw, Cleveland, Kellnersville, Reedsville, Cato, Rockwood, Francis Creek, St. Nazianz, Kiel, Maribel, and ALL surrounding areas for: medical appointments, shopping, nursing home visits, the library, etc.
This program is designed to provide rural individuals access to a range of activities that contribute to their quality of life. The ADRC of the Lakeshore also provides transportation to Manitowoc County elderly or disabled individuals for out-of-county medical appointments. This is a low-cost transportation service. Cost is determined by mileage to and from the appointment based on Federal Mileage Reimbursement rates. TRANSPORTATION – KEWAUNEE COUNTY
The Aging & Disability Resource Center of the Lakeshore contracts with various transportation agencies to help meet your transportation needs based on eligibility.
Kewaunee County is a rural county without a public transit system. As a result, coordinated efforts have been made between the Kewaunee County Department of Human Services and East Shore Industries, Inc. to provide transportation programs to the residents of the county. These programs coordinate transportation rides, utilize volunteer drivers, and work with the specialized transit programs.
Participants who are 55 years of age and older who have no other means of transportation are eligible for services through the Kewaunee County Transportation Program. Service priorities are given to medical and nutrition related activities.
