How Do I Obtain A Vital Record?

As of January 2017 – if you were born in the state of Wisconsin you can obtain your birth certificate from any county Register of Deeds in the State. 

Choose one of the 3 following methods

1. Stop In Our Office:

   • Applicant Will Need their Photo ID or Drivers License
     *OR 3 Pieces of Mail with Applicants Name and Current Address
   • We Recommend bringing Cash. (No Personal Check Accepted)
   • Available Records are Issued Immediately

2. Send Request Through The Mail:

   • Print & Complete Application Form
   • Payment (No Personal Checks): Cash or Money Order will need to be mailed
   • Scanned Copy of the Applicants Photo ID or Drivers License
     *OR 3 Pieces of Mail with Applicants Name and Current Address
   • Records are Issued & Mailed Back the Same Day they are Received

   • Application Forms

     • Birth Certificate Application Form
     • Marriage Certificate Application Form 
     • Death Certificate Application

3. Request For Record Online via VITALCHEK:

   • Visit this link to request record via VITALCHEK
   • Pay with Debit/Credit Card Only
   • An additional Service Fee of $10.00 is applied
   • Has the option to be sent UPS overnight
   • Orders received by 2 p.m. are mailed the same day
