Wellness & Prevention Programs
The ADRC provides access to many evidence-based prevention programs through partnerships with local hospitals, clinics, senior centers and fitness centers. The evidence based prevention programs reduce emergency room visits, hospitalizations and overall health care costs.
Contact any of our offices for a listing of evidence based programs in your area. Visit your local ADRC office to have a conversation with an Information and Assistance Specialist who will provide options on the program that best meets the needs and desires of you or your loved one.
Living Well is an evidence-based workshop for people who have one or more chronic conditions. A chronic condition is an on-going health problem or disease such as diabetes, heart disease, arthritis, and other long-term problems. Classes are highly participative, where mutual support and success build participants’ confidence in their ability to manage their health condition to maintain active and fulfilling lives. It is facilitated by two trained leaders in a classroom style, but most of the learning comes from sharing and helping others with similar challenges. It is a 6 week class (2 ½ hours per week) Designed to improve the self-management skills of those living with chronic health issues. People with various health conditions attend together to learn from and provide support to each other. Scheduled at various times of the year throughout Manitowoc and Kewaunee counties. Stepping On is a program proven to reduce falls and build confidence in older people. Participants gain specific knowledge and skills to prevent falls in community settings. It is designed specifically for people who are:
- at risk of falling
- have a fear of falling
- have fallen one or more times
Workshops are facilitated by trained leaders and provide a safe and positive learning experience. Join us for this 7 week workshop that is proven to reduce falls. Focus is on improving balance and strength, home and environmental safety, vision, and a medication review. Scheduled at various times of the year throughout Manitowoc and Kewaunee counties. Powerful Tools for Caregivers is an evidence-based education program offering a unique combination of elements. The scripted curriculum and the intricately detailed training material, have guaranteed its consistency and quality. The program utilizes a train-the-trainer method of outreach.

The 6-week scripted curriculum has been shown to improve:
- Self-Care Behaviors: (e.g. increased exercise, relaxation and medical check-ups)

Stepping Up Your Nutrition
Did you know that malnutrition can cause muscle loss and dizziness which can increase your risk of falls? If you have two or more chronic conditions, you may be at risk of malnutrition. You may also be at risk for malnutrition if you are losing weight without dieting, not getting enough to eat, losing
muscle strength, and not drinking enough fluids.
