Immunizations are one of the greatest achievements in public health. Vaccines prevent disease in people who receive them and prevent spread to others at high risk for disease so there is little opportunity for an outbreak to occur.

Review School Immunization Requirements
The CDC will no longer allow the Health Department to provide immunizations to individuals with Health Insurance Coverage for Immunizations. If you are not sure if you have coverage, please call your health insurance company directly.
The Health Department holds a free weekly immunization clinic for individuals under the age of 19 who meet one of the following criteria: individuals with no insurance, with insurance that does not cover vaccines (underinsured), who are medicaid eligible, or who identify as American Indian/Alaskan Native .
The following vaccines are available from the health department by calling 920-683-4155 to make your appointment.
Birth through 6 years
- DTaP
- Hepatitis A
- Hepatitis B
- HIB
- Influenza
- MMR
- Pneumococcal
- Polio
- Rotavirus
- Varicella (Chicken Pox)
Teen Vaccines
- HPV
- Influenza
- Meningococcal
- TdaP
Eligible Adults
Adults with no insurance coverage may be able to receive the following vaccines
- MMR
- Varicella (Chicken Pox)
- HPV
- TdaP
Accessing Immunization Records Online
The Wisconsin Immunization Registry (WIR) is a computerized Internet database application that was developed to record and track immunization dates of Wisconsin’s children and adults, providing assistance for keeping everyone on track for their recommended immunizations.
Steps to accessing your record online:
- Go to the WIR website and click on the title Public Immunization Record Access
- In the search screen, enter first and last name
- Enter birthdate using the MM/DD/YYYY format
- Enter ONE of the following*
- Social Security Number
- Medicaid ID
- Health Care Member ID
- Click Search
*Often times, information from step 4 is missing in WIR. The health department or your health care provider can update this information so you can access your immunization record.

Before traveling internationally, check the CDC Travel’s Health Web Page to be Proactive, Prepared and Protected when it comes to your health-and the health of others.
You should schedule an appointment with your doctor or a travel health clinic at least 4-6 weeks before you leave. To find a travel clinic, visit the CDC Web Page.
