Local Forms – Adoption (Step-parent)
Notice of Outstanding Support Order
Manitowoc County Local Court Rules require this form.  If there is an outstanding support order, prospective obligations are terminated when parental rights are terminated.  The information allows the court to notify the appropriate originating jurisdictions of this change in status.

Report of Guardian ad Litem
Petitioner to complete and provide the attorney appointed as the guardian ad litem.  Attorney will complete and file with the court.

Order for Conservator Compensation
The order a conservator should submit with their petition for payment of fees and reimbursement of expenses.

Petition for Conservator Compensation
Conservators may receive compensation and reimbursement of expenses incurred as a conservator.  There must be money available from some known source to make the payment.  This allows the conservator to ask the court for payment.

Delegate Parental Authority
This form is created in the statutes as a way to avoid a legal guardianship of a minor if the child is being placed with non-custodial adult for less than a year.  Court staff is not able to answer questions or provide assistance.  The completed document does not get filed with the court and is used by the caregiver in the parent’s absence.

Order Appointing Examiner
To appoint a doctor and obtain a medical report for necessary guardianship proceeding.

A guardian should submit this order with their petition for payment of fees and reimbursement of expenses.
