Comprehensive Community Services
The Comprehensive Community Services (CCS) Program is a Psychosocial Rehabilitative program for individuals of all ages who need ongoing services for a mental illness, substance use disorder, or a dual diagnosis beyond occasional outpatient care, but less than the intensive care provided in an inpatient setting. The individual works with a dedicated team of service providers to develop a treatment and recovery plan to meet the individual’s unique needs and goals. The goal of this community-based approach is to promote better overall health and life satisfaction for the individual.

- Diagnostic Evaluations
- Medication Management
- Physical Health Monitoring
- Peer Support
- Individual Skill Development and Enhancement
- Employment-Related Skill Development
- Individual and/or Family Psychoeducation
- Wellness Management and Recovery/Recovery Support Services
- Psychotherapy
- Substance Abuse Treatment
Basic Eligibility
- Must be a Manitowoc County Resident
- Be enrolled or eligible for Medical Assistance
- Have a mental health and/or substance use diagnosis
- Be functionally eligible as measured by the State eligibility screen
- Need psychosocial rehabilitation services
